骨鬆、關節退化分不清？物治師來解惑

（優活健康網記者林奐妤／綜合報導）膝關節是人體中很重要的承重關節、也是最大最複雜的關節，不但使用率高，也經常曝露在受傷危險下。現今醫療資訊豐富，從電視網路、報章雜誌取得都非常容易，近年越來越多人積極尋找保護膝蓋的治療方式，然而觀念真的正確嗎？

退化性關節炎、骨質疏鬆病因、治療方式大不同

「走路膝蓋卡卡、上下樓梯會痛」，訴說的症狀較傾向退化性關節炎；「膝蓋常疼痛，自行吃鈣片補充」，即便真的是退化性問題，一般是「軟骨」磨損發炎，鈣片只能補充「硬骨」營養而無用。

衛生福利部桃園醫院新屋分院物理治療師何佳洋提醒，民眾若是從字面解讀，容易混淆專有名詞，其實這「退化性關節炎」、「骨質疏鬆」2種疾病的病因、症狀相差很多，治療方式更不同！錯誤觀念不但白花錢、錯過治療時機，還可能暴露在危險中。

骨質疏鬆如沉默疾病症狀易被當做老化現象

骨質疏鬆症為骨質流失造成骨質密度下降，疾病症狀不顯著，例如背痛、駝背、變矮等，容易跟老化現象重疊，常常骨折才發現，被稱作「沉默的疾病」。然而30歲左右骨質就可能開始流失，偏食缺鈣、少運動和曬太陽更加劇，已是僅次於心血管疾病的全球第2大流行疾病，若造成髖骨、脊椎骨骨折，致死率並不亞於乳癌或是中風。骨質疏鬆症的診斷，一般透過骨質密度檢測是否低於標準值。

退化性關節炎除了基因、代謝因素，也可能因過量使用，導致關節磨損、組織增生，好發年長、女性、肥胖、勞力工作、激烈運動或曾有關節受傷病史的族群。疼痛是最顯著的症狀，從關節僵硬腫脹、活動受限，後期可能長骨刺、軟骨磨損而加重發炎，久站、走路不適，嚴重甚至關節變形、肌肉萎縮，睡覺都不舒服。退化性關節炎的診斷，需透過X光判讀是否有關節腔狹窄或骨刺生成。

退化性關節炎包括侵入治療、飲食、物理治療

骨質疏鬆的治療除了以藥物減緩症狀，非藥物治療可藉由平時注意3大點：「飲食、運動、預防跌倒」。飲食首重鈣質及維生素D攝取，為了有效吸收維生素D還必須有足夠日曬量；運動著重在規律承重和肌力訓練，物理治療師會依個人的生理、服藥狀況，規劃適當的運動方式、強度以及指導。

退化性關節炎治療包括侵入性治療、飲食及物理治療。侵入性治療包括藥物、關節腔注射玻尿酸增加潤滑效果、還有PRP治療，嚴重者可能須接受關節置換手術；飲食則可以多補充葡萄糖胺和軟骨素營養品；物理治療除了常見儀器、徒手治療，輔具選擇、居家衛教和運動設計更重要，物理治療師需根據關節活動度、肌力、肌耐力、柔軟度甚至心肺耐力給予適合的運動處方。

何佳洋物理治療師表示，兩者治療目標都並非治癒疾病，而是藉由整合性的臨床處置，避免更多傷害、改善症狀或減緩疾病進程。預防勝於治療，骨質疏鬆症與退化性關節炎近年都有年輕化趨勢，絕對不可以掛上老年疾病的標籤。如果本身是高風險對象或疾病初期，擁有正確資訊、及早介入治療，才能確保日後生活質量！
